Monitor Guidelines
1. Our office hours
are 8:00am to 5:00pm. Monitoring Hours are 8:30am to 4:45pm.
If additional time is needed, please speak with your coordinator to
see if he or she is available.
2. Upon arrival,
please check in at Suite 309 West. Please do not come directly to
Suite 529. A coordinator will escort you to your assigned
monitoring room. Special requests should be made known in writing
prior to your visit.
3. To respect
confidentiality of our clients, please do not enter the coordinator
work areas.
4. For your
convenience, there is a restroom, telephone, photocopier, and coffee
at your disposal.
5. If you need to
receive calls, please have your party call your mobile phone or our
office at (615) 383-1727.
6. During your visit,
your coordinator will check in with you as frequently as his or her
schedule allows to answer questions and make corrections.
7. The maximum period
to be reserved for consecutive monitoring is 2 days. Exceptions will
be made on a case-by-case basis and should be requested in writing.
8. Should you need to
bring an additional monitor to assist you, this request should be
submitted to the site in writing for approval.
9. For your dining
needs, there is a snack bar and delicatessen on the 2nd floor of the
Medical Plaza. The cafeteria is located on the 1st floor of the
hospital. If you prefer to dine out, your coordinator can provide
you with suggestions and nearby locations.
10. Dr. Pruitt
will plan to meet with you at the end of the business day.
Please schedule your flights accordingly.
11. In our continuing
efforts toward quality improvement, you will be asked to complete a
site visit questionnaire. This information is quite helpful to us in
ensuring that your needs are addressed.
12. Follow-up letters
are expected to be received by our office within 2 weeks of your
visit to enable the CRC to prepare for your next visit.
We would like your visit with us to be
accommodating and enjoyable. If you have any questions please do not
